India's digital health revolution is entering a new phase. As artificial intelligence moves from experimental tools to clinical infrastructure, the country's next healthcare transformation will depend on integrating AI into diagnostics, public health, hospitals, and preventive care—while ensuring trust, equity, and robust governance.
What happened
India's healthcare transformation is entering a new phase. After building one of the world's largest digital public infrastructure ecosystems through Ayushman Bharat Digital Mission (ABDM), Ayushman Bharat, CoWIN, eSanjeevani, and expanding digital health records, policymakers are increasingly turning towards artificial intelligence to improve diagnostics, hospital operations, disease surveillance, drug discovery, and personalised healthcare.
The shift reflects a broader recognition that India's healthcare challenge is no longer only about expanding hospitals and doctors, but about improving efficiency, accessibility, and preventive care across a population exceeding 1.4 billion.
Why it matters
ndia's healthcare system remains under significant structural pressure.
Government health expenditure remains around 2% of GDP, below many emerging and advanced economies. The country has roughly one doctor for every 800–900 people—meeting WHO benchmarks overall but with stark rural-urban disparities—and specialist shortages persist across districts. Non-communicable diseases such as diabetes, cardiovascular illnesses and cancer now account for the majority of deaths, while the elderly population is projected to nearly double over the next three decades.
Traditional healthcare expansion alone cannot close this gap.
Artificial intelligence offers an opportunity to shift India's model from reactive treatment toward predictive, preventive and personalised healthcare.
Globally, AI is already assisting radiologists in detecting cancers earlier, identifying diabetic retinopathy from retinal scans, accelerating drug discovery, predicting disease outbreaks, automating hospital administration and supporting clinical decision-making. McKinsey estimates that AI could create hundreds of billions of dollars in annual value across healthcare worldwide through improved productivity, diagnostics and operational efficiency.
India is particularly well positioned because it has already built digital public infrastructure that many countries still lack.
More than 550 million Ayushman Bharat Health Accounts (ABHA) have been created. eSanjeevani has facilitated hundreds of millions of telemedicine consultations. Digital vaccination infrastructure demonstrated during COVID-19 showed that healthcare delivery can scale rapidly when supported by interoperable digital systems.
This creates a rare opportunity.
Rather than building AI around fragmented hospital networks, India can integrate AI into a nationwide digital health ecosystem where electronic health records, diagnostic platforms, insurance systems and telemedicine services increasingly communicate with one another. However, becoming an AI-native healthcare system requires solving problems beyond technology.
Healthcare data remains fragmented across public and private providers. Data quality varies significantly between institutions. Many district hospitals lack adequate digital infrastructure, cybersecurity capabilities and trained personnel. AI systems trained primarily on foreign datasets may underperform for India's diverse population unless domestic clinical datasets improve substantially.
Regulation will also determine public trust.
Clinical AI cannot operate under the same principles as consumer software. Transparency, explainability, patient consent, algorithmic auditing, medical liability and cybersecurity safeguards will become essential as AI assumes larger roles in diagnosis and treatment. Without strong governance, technological progress could increase inequity instead of reducing it.
India also has an opportunity beyond domestic healthcare.
The global digital health market is projected to exceed US$1 trillion during the next decade. Combining pharmaceutical manufacturing, health-tech startups, AI research and digital public infrastructure could position India not only as a healthcare provider but also as an exporter of affordable AI-powered healthcare solutions for developing economies.

The second leapfrog is fundamentally different.
It seeks to make healthcare intelligent rather than merely digital.

Countries including the United States, China, Singapore and the United Kingdom have already incorporated AI into national health strategies. India's advantage lies in combining population scale with digital public infrastructure—a combination few countries possess.

The next phase of India's healthcare strategy is likely to focus on integrating AI with the IndiaAI Mission, expanding digital health records under ABDM, improving interoperability across healthcare providers and encouraging responsible AI adoption through regulatory standards.
The larger question is no longer whether AI will enter healthcare. It is whether India can become the world's first major economy to build an AI-native public health infrastructure—one where prevention, diagnosis, treatment and health management are increasingly supported by intelligent digital systems operating securely at population scale.
